The Social Coach is responsible for ensuring the health, safety and ongoing supervision of residents and contributors receiving residential and/or day service supports in their community-based home.
The Social Coach is responsible for assisting Residents and Contributors in the fulfillment of their ISP outcomes, when applicable, and supporting the health, safety and the personal growth of the resident and/or contributor.
Essential Duties and Functions
- Read and assist in the implementation and development and of the ISP (when applicable)
- Provide residential supports to consumers according to the assessment and ISP (when applicable)
- Assist consumers in the completion of their outcomes and residential goals.
- Implement supports developed from plans written by the therapists.
- Remain familiar with diagnoses of consumers within the home.
- Assist consumers with the development of a healthy and respectful culture.
- Assist the consumer to effectively engage in therapeutic/healthy relationships.
- Enter and submit work-time in HomeBase on a regular basis and before deadline
- Communicate with staff at shift change about the status of the consumers and in a timely matter.
- Check and communicate via PCC email as directed.
- Supervise/assist in menu planning and meal preparation.
- Assist/supervise the consumers when they are completing home maintenance tasks.
- Transport consumers to all appointments, activities and community events.
- Provide access to recreational and leisure activities in the home and community as well as encourage participation in activities.
- Write thorough and effective communication, progress reports and complete data sheets.
- Administer medications to consumers as prescribed (after completing approved training).
- Provide medical emergency procedures as needed.
- Identify and report any residential environmental deficiencies ASAP to the Executive Director
- Maintain a valid driver’s license and drive safely in all environments.
- Respect consumer’s culture and life choices.
- Perform crisis intervention techniques when necessary.
- Assist/support resident in maintaining good hygiene (i.e. tooth brushing and hair care).
- Identify and report incidents to Executive Director immediately; write and submit incident report the same day that the incident is identified.
- Attend all mandatory site staff meetings.
- Perform all other duties as assigned by your supervisor.
Qualifications:
1. Must be at least 18 years of age
2. Have a High School Diploma or equivalent
3. Completed all required training
5. Successful completion of CPR & First Aid training required
6. Subject to criminal background check
8. Successful completion of bi-annual physical with TB test
Compensation: $20.00 per hour

We know society has misjudged people we call Contributors: people who are different intellectually/developmentally, and may negotiate mental health challenges.
In recent years, Partners Creating Community established three Phoenixville-based businesses. Green Lion Breads with Founder and Head Baker, Mark Doberenz (2017) and Heart Stone Coffee with Peter Mike-Mayer (2018) both operate in Franklin Commons on the north side of Phoenixville. In 2019, Partners Creating Community opened Frog Hollow Farm, a therapeutic residential farm home for Contributors, and in 2020 they established Pear Tree Coffee Roasters, a coffee roasting business operating at the farm.
Partners Creating Community’s goal is to offer a holistic approach to serving individuals of all abilities. This approach includes a safe and stimulating farm home, meaningful work that meets real community needs, and educational and vocational programs that provide opportunities for Contributors to become more active and engaged members of their communities.

We support them by providing residential care and acquiring and developing an appropriate community space in which to house and conduct educational and vocational programs. Partners Creating Community fosters holistic health and healing by developing creative programming, hosting several events and festivals, and collaborating with innovative, like-minded social, therapeutic, and artisan enterprises to further support the vocational and educational needs of Contributors.
